The Chuck McBreen Basketball Camp.....

| 1. Fill out the Registration Form. |
| 2. Print and mail the form along with a $100 deposit (Before July 1st) for each week of camp. Applications postmarked after July 1st must be accompanied by payment in full. Payments mailed one week or less prior to the first day of camp week, must be made by certified check or money order. Make checks payable to: C.J.M. Hoops, Inc.

| 3. Medical Forms Needed for Paticipation: The Township of Mahwah, and the State of New Jersey certify the Chuck McBreen Basketball Camp. A doctor physical that includes all records of immunization must be on file with the camp prior to the first day of the camp session. There will be no exceptions due to county and state inspections. |
